The nominee must be an Ontario resident who has contributed to the community after age 65.
To qualify, they may have contributed to the arts, volunteering, education, fitness or humanitarian activities.
The deadline for nominations is Dec. 1.
Since 1987, the Ontario Senior Achievement Awards have recognized 676 outstanding seniors.
